Complex Discharge Team at London North West University Healthcare NHS Trust to adopt Infinity
London North West University Healthcare NHS Trust has announced that the Infinity platform will be adopted by Northwick Park Hospital’s Complex Discharge Team in a transformation project replacing an outdated database and paper lists.
The digital task management platform enables staff to log, share and coordinate their work in real-time, and will reduce reliance on phone calls, instant messages, and paper notes to share information.
Northwick Park Hospital’s Complex Discharge Team looks after approximately 6,000 patients a year, whose circumstances mean that leaving hospital is not straightforward. This might be because a patient needs to be cared for in a nursing or residential home, is homeless, or requires services from the local authority or others. The Complex Discharge Team works closely with organisations outside the trust to ensure that when patients leave hospital, they go to the right place for them.
Using Infinity will give staff real-time information about patients, making it easier to coordinate work and share updates appropriately. Staff will be able to access Infinity from anywhere using their smartphone, tablet, or desktop computer, making it much more practical than the existing paper and database system.
This project is the fifth between the trust and Infinity Health.
